Your Vaya Total Care Team

The people who support you on your way to better health make up your Vaya Total Care Team. This can include loved ones and friends, as well as peer support specialists, nurses, licensed counselors, pharmacists or physicians. Our care coordinators will talk with you about your healthcare goals and help you identify the team of people who can best support you along the way.

Who is my care coordinator?

Vaya Total Care members have a personal care coordinator who works closely with them to identify their strengths, identify their needs and build a plan of care that is right for them. We also support wellness and recovery through personalized outreach and care planning. Our aim is to make it easier for you to access the care you need so that you can achieve your healthcare goals.

Your care coordinator will work with you and your loved ones, as well as your doctors and other healthcare providers, to identify the supports that may be helpful to you. He or she will also help you identify your individual strengths, as well as the things that are important to you. The information that you provide will help shape your individualized, person-centered care plan and will guide us as we work together to meet your healthcare goals.

Who is on my care team?

You and your loved ones will decide who you want on your team. Your team may include your parents, legal guardians, care coordinators, behavioral health or IDD providers, physicians or other medical professionals, family, friends, acquaintances and other community members who can support you on your way to better health.
