Entre problemas de salud mental y la separaci\u00f3n de su pareja, Jody hab\u00eda abandonado una de sus mayores pasiones, el arte, y no dejaba de preocuparse por los pr\u00f3ximos pasos en su vida, como d\u00f3nde vivir.<\/p>\n\n\n\n<p>Fue entonces cuando intervino el equipo de la Iniciativa de Transici\u00f3n a la Vida Comunitaria (TCLI) de Vaya Health. Trabajando en estrecha colaboraci\u00f3n con Jody y otros miembros del personal de Vaya, el equipo ayud\u00f3 a Jody a mudarse a una casa propia. Hoy, Jody conduce su propio coche para ir y volver de las citas -lo que antes era una fuente importante de ansiedad- y dedica tiempo cada d\u00eda al arte, que vende tanto a nivel local como en Internet.<\/p>\n\n\n\n<p>Durante los 12 meses que finalizaron en junio de 2020, Jody fue uno de los 82 nuevos miembros alojados a trav\u00e9s del programa TCLI de Vaya. Ese n\u00famero super\u00f3 el objetivo de 67 establecido por el Departamento de Salud y Servicios Humanos de Carolina del Norte, y durante una pandemia. El a\u00f1o anterior, Vaya super\u00f3 el objetivo estatal de 61, con 65 nuevos miembros alojados.<\/p>\n\n\n\n<p>TCLI, creado a trav\u00e9s del acuerdo de liquidaci\u00f3n de 2012 del estado con el Departamento de Justicia de EE.UU. (DOJ), conecta a los adultos con enfermedades mentales graves a los servicios de salud y apoyos que les ayudan a mantener un hogar en su propio nombre, en lugar de vivir en un centro. Vaya gestiona el programa en 22 condados del oeste de Carolina del Norte. El personal de Operaciones de la Red de Proveedores es clave para garantizar la participaci\u00f3n de los proveedores. La L\u00ednea de Crisis de Salud Mental de Vaya gestiona el proceso de derivaci\u00f3n, y el Equipo de Vivienda se encarga de la comunicaci\u00f3n con los propietarios, identifica las vacantes y gestiona una base de datos de contratos de arrendamiento. Una vez completada la transici\u00f3n, se asigna a los participantes un gestor de cuidados de Vaya para la coordinaci\u00f3n continua de los servicios y ayudas.<\/p>\n\n\n\n<p>El n\u00facleo del programa dentro del Vaya es el Equipo TCLI.
