High Country Care Transitions Summit

Care transitions refers to someone moving from one care setting to another. One of our goals, as a network of providers, is to improve the effectiveness of these transitions. This event is designed to share effective practices at the national, state and local levels, highlight available resources and develop and strengthen partnerships. The day will include plenary speakers, breakout sessions and networking time, providing multiple opportunities to participate and collaborate.
