How does Vaya Total Care work?
Vaya Total Care is designed to help people with mental health or substance use challenges, or intellectual or developmental disabilities (IDD), who may also need medical and social support. Our team of dedicated professionals is proud to serve Vaya Health members age four to 65 who have complex healthcare needs or who have recently experienced serious changes in their life or in their health.
At Vaya Health, we know that many things can affect your health – from medical conditions or mental health concerns to housing, food and employment needs. That is why our team will work closely with you and your loved ones to identify your goals and focus on real solutions to your healthcare challenges. Improving health means improving our ability to identify your needs and help you find the right supports at the right time. Our care managers will listen to you and help you connect with the care you need.

HOW DOES VTC WORK?
What matters to you is what matters to us. That is why we work closely with you to make sure that your voice is heard and your needs are understood. You will be asked to let us know what goals are most important to you and your family, and, together, we will create the plan that is right for you. Our care managers are available to answer your questions and can offer tips and more.
