Vaya Health believes having a safe, stable place to live is a critical part of well-being and recovery. Our Transition and Housing Department focuses on improving quality of life for individuals who qualify for the specialty programs listed below. Housing specialty programs are not an entitlement, and funds are limited.
Vaya places emphasis on social determinants of health, including economic stability and neighborhood/built environment. We do this by:

Housing support line
Vaya’s Housing Support Line at 1-800-893-6246, ext. 4560, connects members and recipients experiencing homelessness or housing insecurity to appropriate resources. Call or email HousingTeam@vayahealth.com to request support.
Housing programs
TCLV is North Carolina’s tenant-based rental assistance for eligible TCL participants seeking community-based supportive housing.
PSH is a combination of housing with indefinite leasing or rental assistance, paired with supportive services to help individuals who are unhoused, people with a disability, and families with an adult or child with a disability live as independently as possible.
Non-Medicaid Residential Services are allocated in a manner that is fair, consistent, and intended to help meet the medically necessary residential placement needs of eligible individuals who are indigent, uninsured, or under-insured.
Los fondos de la Subvención de Apoyo a la Vivienda se utilizan, en la medida de lo posible, para ayudar a establecer viviendas menos restrictivas para los miembros adultos y beneficiarios que reúnan los requisitos y cuyos ingresos brutos sean iguales o inferiores al 100% de las directrices federales de pobreza para una sola persona.
The Vaya Independence Project is a combination of housing with short-term rental assistance and supportive services to help members and recipients with complex health needs live as independently as possible.
Back@Home programs are designed to reduce unsheltered and rural homelessness and improve services, health outcomes, and housing stability for highly vulnerable unsheltered individuals and families. The programs offer supportive services, temporary housing, permanent housing, unit access, financial assistance, move-in financial assistance, and rent or utility assistance.
Vaya's Back@Home Balance of State program includes Alamance, Alexander, Caldwell, Chatham, Franklin, Granville, McDowell, Person, Rockingham, Rowan, and Vance counties. Back@Home Helene covers Buncombe County.
Vaya’s statewide MFP Transition Coordination program helps older adults and people with physical disabilities who live in facilities move back into the community.
Vaya’s MFP I/DD In-Reach and Transition program helps people with I/DD who live in facilities move back into the community.
The Justice Reentry Program supports eligible individuals who are being released from prison and returning to a county in the Vaya region by connecting them to needed services and supports.
The Integrated Supportive Housing Program is managed by the North Carolina Housing Finance Agency. The program provides interest-free loans for independent apartments where no more than 20% of the units are integrated and set aside for people with disabilities.
