We know that more goes into your health and well-being than what happens inside your doctor’s office. Our program, Vaya Total Care Perks, is here to support you. These value-added services are available to qualified members at no cost, to support you in living the life you choose.
To be eligible for these perks, you must be a member of Vaya Total Care with Medicaid. Each perk may have additional eligibility requirements. Vaya Total Care Perks are not available to Vaya Total Care state-funded recipients and NC Medicaid Direct beneficiaries.
Wellness Perks
We know that maintaining a well-balanced diet after leaving the hospital can help you heal and recover. For Vaya Total Care members who are discharged from a qualifying inpatient hospital stay, we offer a meal delivery perk from Mom’s Meals.
Eligible members may receive two free home-delivered meals per day for up to seven days in the first month after discharge. This perk is only available during the 30 days following the hospital discharge date.
To be eligible, you must:
- Be a Vaya Total Care Medicaid member
- Be participating in Tailored Care Management
- Not be eligible to receive meal delivery through the NC Innovations Waiver
- Have been discharged from a qualifying inpatient hospital stay within the last 30 days
Outpatient surgeries performed at a hospital and visits to the emergency department do not qualify.
To request this perk, contact your care manager during a qualified hospital stay, after you have been discharged from a qualified hospital stay, or before you expect to have a qualified hospital stay.
Vaccinations are important to protect you from getting and spreading preventable diseases. Eligible members can get a $10 gift card for receiving the COVID-19 vaccine, including boosters, or influenza (flu) vaccine. This perk can be used for up to two gift cards per plan year. You may choose a gift card from Amazon, Target, or Walmart.
To be eligible, you must:
- Be a Vaya Total Care member receiving Medicaid.
- Have proof of receiving a COVID-19 or influenza (flu) vaccine during the current plan year.
This perk is available to members of all ages.
To request this perk, submit your request and proof of vaccination through the Member and Recipient Portal. After your request is reviewed and your eligibility is approved, the gift card(s) will be sent to you.
The WW (formerly Weight Watchers) program offers a mobile app to work on building healthy eating patterns and weight loss skills. Eligible Vaya Total Care members will receive a voucher for a 6-month WW mobile app subscription. This perk can be received one time per calendar year.

- Barcode scanner
- Wellness Wins rewards program
- Meditation content
- 5-minute coaching audio lessons
To be eligible, you must:
- Be a Vaya Total Care member receiving Medicaid.
- Be at least 18 years of age.
- Have a body mass index (BMI) of 25 or higher.
To request this perk, submit your request and medical documentation that confirms a BMI of 25 or greater through the Member and Recipient Portal. After your request is reviewed and eligibility is approved, a voucher code will be sent to you via email.
*The WW program vouchers do not include WW workshops.
Pregnancy Support Perks
Vaya Total Care members may receive a free breast pump from our vendor, Aeroflow. On the Aeroflow website, members may choose a pump from a list of options. This perk can be used within 365 days of the first prenatal visit.
Additionally, members will also be able to receive education and support through Lactation Link by Aeroflow Breastpumps.
To be eligible, you must:
- Be a Vaya Total Care member receiving Medicaid.
- Have attended a prenatal visit within the first 16 weeks of gestation.
- Be within 365 days of your first prenatal visit.
To request this perk, make a request on the Aeroflow website or call Aeroflow at 1-844-867-9890.
Education Perks
Congratulations on starting your journey to receive a high school equivalency diploma! Vaya Total Care members can receive an education voucher. This voucher will provide eligible members access to:
- One-on-one guidance from a bilingual advisor
- Top rated study materials that are available in English or Spanish
- -Ready practice tests and the tests
- Expert tutors for students who need additional help
- A test-pass guarantee with unlimited access to the program until the student has passed the
- Private online community support group for students through Facebook
- Ability for students to earn college ready credits
The voucher does not depend on exam results.
To be eligible, you must:
- Be a Vaya Total Care member receiving Medicaid.
- Be at least 18 years of age.
- Not have completed or received a high school equivalency diploma
To request this perk, start the process on the GEDWorks™ website. You will be asked to enter your information so that your eligibility can be confirmed. If approved, you will work directly with Works™ to get a brief introduction to the platform and support.
Other Perks
Vaya Total Care members with an intellectual/developmental disability (I/DD) may be able to receive a discount toward the purchase of home safety and assistance equipment. The $100 discount can be used toward a purchase from a list of options. This list includes items that are not currently reimbursable as durable medical equipment (DME).
Some examples of eligible products are:
- Home sensors to provide prompts to members
- Environmental controls to assist members with limited mobility
- Electronic medication dispensers
- Personal emergency devices
- Video monitoring equipment
To be eligible, you must:
- Be a Vaya Total Care member receiving Medicaid.
- Be participating in Tailored Care Management.
- Have an intellectual/developmental disability diagnosis.
- Be participating in independent living services (consult with your care manager to see if your service qualifies) .
A member does not need to be receiving NC Innovations Waiver services or be on the Registry of Unmet Needs.
To request this perk or learn more, contact your care manager. They will review your eligibility and schedule a consultation with Simply Home to understand your individual needs and recommend products that qualify for the discount.
Vaya Total Care members may receive up to 24 trips each year from our transportation vendor, Modivcare. This transportation perk is to help you get to health and non-health related destinations, such as:
- Libraries
- Prenatal classes
- Grocery stores, food banks, and farmer’s markets
- Post offices, FedEx, and UPS
- Notaries, legal aid, and attorneys
- Parks, walking trails, and gyms for exercise
- Local community activities and job-related activities
- Banks or places to pay bills
Each trip is considered a one-way trip and can be up to 50 miles away from your pickup location. Traveling to and from one location would be two trips and have a maximum distance of 100 miles roundtrip.
For transportation to a pharmacy, the one-way trip can only be up to 10 miles away from your pickup location with a maximum of 20 miles round trip.
The Transportation Perk is separate from our Non-Emergency Medical Transportation (NEMT) benefit. It is available through the same provider.

- Be a Vaya Total Care member receiving Medicaid.
To request this perk, call Modivcare at 1-888-621-2084 between the hours of 7 a.m. and 6 p.m., Monday through Saturday. Modivcare can help you choose the option that best fits your needs. Rides must be scheduled at least two days in advance.

Vaya Total Care Perks are intended to complement existing supports, not to replace or duplicate them. Members cannot receive a Vaya Total Care Perk if they are eligible for the same support through a formal, established service or through their NC Innovations Waiver Individual Budget. In general, members residing in congregate care or institutional settings are not eligible for Vaya Total Care Perks. Vaya reviews all requests for member eligibility and to ensure Vaya Total Care Perks do not duplicate available supports.
