Rural Organizations Orchestrating Transformation for Sustainability
The NCRHTP is a statewide initiative supported by $213 million in federal funding approved by the U.S. Centers for Medicare & Medicaid Services. Program funding is designed to address longstanding challenges in rural health care, including workforce shortages, hospital sustainability and gaps in access to mental health and primary care services. Over the next five years, the program will support more than 400 rural health facilities across North Carolina and bring hundreds of millions of dollars in federal investment to the state.
NC ROOTS Hub for Region 3
The North Carolina Department of Health and Human Services (NCDHHS) has selected Vaya to serve as NC ROOTS (Rural Organizations Orchestrating Transformation) Region 3 Hub Lead (see map below). We are currently working with community partners to build a solid foundation for coordinated regional collaboration to support the program.
This page will serve as a central location for program updates, partnership and participation opportunities, stakeholder communications, meeting announcements, and resources for communities and providers.

Questions and answers
A key component of the NCRHTP is the launch of locally governed NC “Rural Organizations Orchestrating Transformation for Sustainability” (ROOTS) Hubs, which will serve as regional networks connecting medical, behavioral health, and social supports. Vaya is the ROOTS Hub Lead for Region 3. Statewide, there are six regional Hubs.
Region 3 consists of the following 12 counties: Alexander, Anson, Cabarrus. Catawba, Cleveland, Gaston, Iredell, Lincoln, Mecklenburg, Rowan, Stanly, and Union.
The Hub will support multiple initiatives focused on improving regional health outcomes, strengthening partnerships, and advancing coordinated community-based strategies.
Appalachian State University and Partners Health Management serve as Vaya’s key partners at the decision-making level of the Region 3 ROOTS Hub.
Local input and expertise are critical to the success of the NCRHTP. Vaya is currently developing formal engagement and intake processes. Organizations interested in participating are encouraged to join our mailing list to receive updates and information future opportunities by email.
Vaya will share information about future funding, partnership opportunities, and participation models as implementation planning progresses.
In addition to this webpage, Vaya will share program announcements through our email updates, regional partner communications, and community meetings and engagement opportunities. We encourage providers, community organizations, local leaders, advocacy groups, and other stakeholders interested in supporting regional health improvement to subscribe.
