Vaya Health Members Share Their Unique Journeys in a Newly Released Video

Spotlighting the importance of individual journeys from institutional to independent living

Vaya Health, a leading regional managed care organization dedicated to improving the lives of individuals with behavioral health, intellectual/developmental disabilities (I/DD) and traumatic brain injury, releases a powerful new video that showcases the life-changing experiences of its members involved in the “Peer-Extender” Program.

The Peer Extender Program, which is a part of Vaya’s Transitions to Community Living program,  is an initiative designed to support individuals on their journey from institutional settings to integrated community living. The program provides a holistic approach to care, showing how Vaya meets members where they are on their path to recovery and supports them living the life they choose. The program offers members access to a range of services, including personalized care coordination, peer support, housing assistance and more. By focusing on individualized plans and fostering community connections, Vaya helps empower program participants to achieve independence.

In the newly released video, viewers witness firsthand the remarkable stories and heartfelt testimonials of program participants whose lives have been profoundly impacted by the program. The video features interviews with members who have successfully transitioned to community living, as well as support professionals who have played instrumental roles in their journeys.

“It gives me a light at the end of the tunnel” shared Paula, a Vaya member, while speaking about working with a peer extender specialist.

The video not only sheds light on the individual success stories but also emphasizes the collaborative efforts of Vaya’s network of community partners, whose contributions are essential to the program’s success.
