Primary Care Provider – Choice Period

The primary care provider (PCP) choice period is now open for Medicaid beneficiaries enrolled in Vaya Health’s Behavioral Health and Intellectual/Developmental Disabilities (I/DD) Tailored Plan (Vaya Total Care). As a Tailored Plan member, you or your guardian may choose or change your PCP during the PCP Choice Period from April 15 – May 15, 2024 or you will be auto-assigned a provider.

To find available PCPs in your area:

• Visit the Find a Provider page of Vaya’s website.
• Select “Click here to search” in the “Find a Provider” tile on the lower left side of the screen.
• Enter your ZIP code. 
• Select “Primary Care” from the dropdown menu under “Search by Service Category” or, if you are seeking a specialist, select the correct specialty from the dropdown menu under “Search by Specialty.”
• Select “Search” and a list of PCPs will appear that shows their distance from the ZIP code entered.

For assistance in choosing or changing a PCP, you may speak with your assigned Vaya care manager, call Vaya’s Member and Recipient Service Line at 1-800-962-9003 (7 a.m.- 6 p.m., Monday-Saturday, including holidays), or complete the PCP Change Request Form.
